Lowering an Overly High Hairline

There are two techniques for lowering an overly high hairline — hair transplantation or hairline lowering surgery (aka forehead reduction). Dr. Demetri is one of the few facial plastic surgeons in the country who performs both.

Hairline lowering surgery takes place under anesthesia so our patients do not have any pain or memory during the procedure. It only takes about an hour and a half during which Dr. Demetri is able to lower the hairline anywhere from 2-4 cm. One of the main benefits of this surgery is the immediate result. During your consultation, Dr. Demetri will evaluate your hairline to ensure you have a flexible scalp to allow for robust advancement. Furthermore, he will check to see if any significant hair thinning or loss is present which may indicate you are not an ideal candidate. On the other hand, hair transplantation (aka hair grafting) can be done to insert hair follicles at the hairline to lower it in a natural direction. After your evaluation with Dr. Demetri, you can then decide which option is best for you.

Am I a Good Candidate for Hairline Lowering Surgery?

The best candidates for hairline lowering surgery are women with a large forehead. The procedure balances the facial features of those who find their foreheads to be overly large compared to the rest of their face.

If you are experiencing active hair loss, then you are not a good candidate. In this situation, if surgery were to be undertaken, the incision site of hairline lowering surgery would eventually be revealed. Because of this, these patients can instead benefit from solutions like platelet-rich plasma (PRP) injections or follicular unit extraction (FUE) hair transplants.

How to Prepare for Hairline Lowering Surgery

A week before the procedure, avoid blood-thinning medications, anti-inflammatory drugs, and herbal supplements as directed. This will help to prevent excessive swelling or bruising.

Before undergoing hairline lowering surgery, avoid smoking and drinking alcohol for at least two weeks. These activities can have a negative effect on blood flow and the healing process.

Our team will provide a list of items and medication to help you recover properly after the procedure. This list may include pain medication, antibiotics, vitamins, ice packs, and other cleaning supplies. Dr. Demetri will guide you on the schedule of each medication and how to use the aftercare supplies.

Technique

During hairline lowering surgery, Dr. Demetri makes a precise incision at the front of the hairline. Then, the entire scalp can be advanced forward as the excess forehead skin is removed. As you recover, the incision heals in a manner so that hairs actually regrow through it!

Dr. Demetri most commonly performs this under general anesthesia. Your recovery is actually easier than expected, most patients only need pain and nausea pills the first night following surgery. Patients can shower 48 hours after the procedure. It is highly recommended to use mild hair products.

Avoid using a hair dryer, and refrain from coloring your hair for at least two months. Do not expose the incision sites to direct sunlight to prevent discoloration. Use the correct sunscreen to protect your skin.

You return about 7 days later to have the sutures removed. The full results of hairline lowering surgery can be seen after 6 weeks. In most of our cases, Dr. Demetri lowers the hairline by 2 to 4 cm which can make a dramatic difference.

Following surgery, all patients should be aware that in 4-6 months we can still perform an FUE hair transplant to the scar for disguise and increased density. This is a great adjunct to further enhance your results.
